The chapel was built in the 14th century. The chapel was located outside the city walls because it was part of a leper hospital complex, which were built outside the city limits. It was funded by the Trzebiatów City Council. It is a brick hall church built on a rectangular plan, with a pentagonal chancel. In the 19th century, the hospital buildings were demolished, and the chapel itself was plastered and converted into a residential house. The historic site can only be viewed from the outside.
