The chapel is a remainder of the 13th-century church which was demolished in the 19th century. In 2009, the chapel underwent a renovation and now serves as the Tourist Information Centre.

The gothic chapel, located in the oldest part of the town at Bolesława Chrobrego Square, is an outstanding reminder of the thirteenth / fourteenth-century St. Mary's Church, which had existed in this place until the end of the nineteenth century, and was later demolished. The chapel was located by the southern wall of the temple. The base of the chapel is a stone foundation of boulders. It was built of red brick and covered with a gabled roof. The entrance is on the southern wall. The heavy stylised door is embedded in a ogival portal ornamented with black and red glazed brick fitting laid alternately. On the right side, there is a ogival window. There are three ogival blinds, which are shallow niches in the wall, of the recess of an arcade with decorative brick fillings. In the eastern and western elevation, there are single windows between two ogival blinds. Above, in the eastern elevation, there are three additional blinds with a decorative geometric pattern - the so-called blind tracery. In the western elevation there are four additional blinds, but they are without ornamentation. In 2009, the chapel was renovated; You can admire it illuminated in full glory in the evening.  Throughout its history, the chapel has served as a morgue at a time when a church cemetery was here. There used to be also an altar here. The chapel served also as a church porch. At the beginning of this century, it housed a jewellery store, and currently the building serves as the Tourist and Culture Information Centre that is subordinate to the Municipal Cultural Centre in Police. Near the chapel, on the well-developed and well-kept green area, a small model of St. Mary's church was placed. The church was there until the end of the nineteenth century, and the chapel, preserved until contemporary times, is its part.
